Intermediate Care Facilities (ICF/ID)

Individually licensed homes in the community supporting six to eight adults. Direct Support Professionals, working on a shift basis, provide active treatment teaching new self-help skills and preventing individuals from losing their current skills. Staff support is provided 24 hours a day, seven days a week. The staff teaches skills that assist each individual in developing or maintaining their self-dignity and functioning more independently in the community.

Admission to our ICF/ID homes is initiated through the county referral process. Contact your Supports Coordinator for more information on the residential referral process.
